FINAL FRCA Short Answer Questions Club

A free revision aid for those planning to sit the Final FRCA Short Answer Questions exam.

The Short Answer Questions (SAQs) are a daunting component of the Final FRCA exam. To aid revision for this exam, the North Central School of Anaesthesia is offering you the chance to join the Final FRCA Short Answer Questions Club (FRCASAQS). 

Feedback from the first round has been universally positive:

- SAQ writers club was single greatest motivator for revision. It was the one weekly activity I strangely enough looked forward to!

- Fantastic to be able to compare my responses to my cohorts, and it motivated me to face the books, when nothing else did!

- Really very useful.  Good in the early stages for motivation, and later on for practising more time pressured responses and expanding knowledge, as well as having a body of answers to compare any points that were unclear.

What is the aim of the Final FRCA Short Answer Questions Club?
The primary aim of the FRCASAQS is to help you revise for your exam. Whether you are just starting off with your revision, or well into it, the FRCASAQS is here to help. Whilst there is no substitute to hard work, we hope we can motivate you in your studies with carefully chosen practice questions, feedback on how your peers are doing, and the opportunity to sit a practice exam prior to the big day.

How does the FRCASAQS work?
1) At the start of every week, a number of carefully selected SAQs will be sent to your email account. These questions will have been specifically chosen, encompassing examiners favourites, previously poorly answered questions, and current topics of interest.  As the exam draws nearer, the number of questions sent out each week will be increased.

2) At a convenient time in the week, you should write answers for the set questions with pen and paper, laying out the answers exactly as you would in the exam. How you go about this, how you lay out your answer, and whether you read up on the topics prior to answering them, is totally up to you.  You know, or will soon work out, what is best for you.

3) Once answered, scan your answer sheet and then email them back to us at (There are lots of free apps you can download onto your phone to do this e.g. Scanner Mini (by Readdle)).

4) At the end of the week, we collate and anonymise all the answers (scans) we have received, and then email them back out to everyone along with the next weeks questions.

5) If you have previously replied with answers, you will therefore be in the position to look over other peoples responses, which in turn will:
o Allow you to consolidate, and even boost your knowledge on the topic.
o See how others answer the questions in terms of their layout, style, prioritisation of information etc.
o Motivate you in your exam revision.

6) And so the process continues

7) Finally, in the run up to the exam, there will be a chance to sit a practice exam. Twelve questions in three hours under exam settings a task well worth attempting prior to the big day.

It is totally up to you to reply to the set questions, and read your peers responses, however you will very much get out of the process, what you put in to it.  Similar methods to this have certainly boosted the SAQ pass mark in a number of other deaneries around the country, and we hope this will be the case here in North Central London.

How do I join the Final FRCA Short Answer Questions Club?
THE FRCASAQS is totally free, and all you have to do is send an email to to register. We will then add your email address to the weekly mailing list and thats you good to go.  Emails will continue to be sent to you until you ask to be removed from the list which hopefully will not be too long after you join the FRCASAQS when you pass the SAQs exam!

Primary FRCA Masterclass

Dear College Tutor,

The next Primary FRCA Masterclass will take place at the RCoA from Tuesday 10th to Friday 13th of January 2017. Please note that this course is Tue-Fri rather than our usual Mon-Thu.

The course is a mixture of lectures and practice MCQ papers enabling candidates to compare their performance against their peers. The course only runs twice a year and is primarily aimed at candidates planning to sit either the March Primary MCQ Paper although anyone is welcome to attend.

Many congratulations on joining the North Central School of Anaesthesia for August 2016.

I am the training programme director for core trainees in the North Central School (have recently taken over from Dr Marchant)

This is a brief message to welcome you to the school. I hope it will be educationally fulfilling, successful and enjoyable.

If you have any problems during your placements, I would encourage you to discuss these with your educational supervisor and or college tutor at your base hospital in the first instance. Failing this, or if you do not feel comfortable then do contact me directly

I am based at The Royal Free Hospital and you can always make an appointment to see me if there is anything you would like to discuss in person.

I believe most hospitals will be setting up a mentoring system run by SpRs as an additional point of support

Please make sure that you are able to access the Central School of Anaesthesia website http://www.schoolofanaesthesia.co.uk/ - lots of useful information regarding the hospitals in our school, your rotation details, ability to send emails to other members in the school, updates regarding teaching etc.

As you may know, we will be running a novice course and core trainee teaching days. You should be notified of these shortly if you have not already been. Novices who have already completed their basic competencies previously are welcome and expected to attend this too.

Perioperative Medicine implementation - Anaesthetics 2010 Curriculum

Dear all,

To support the introduction of the perioperative medicine (PoM) units of training, the following implementation plan has been agreed with the GMC -

Trainees and trainers should implement this unit of training when trainees commence their next respective level of training (i.e. intermediate, higher);

• CT1 Core unit from August 2016
• ST3 Intermediate unit from August 2016
• ST5 Higher unit from 2016
• ST6/7 Advanced unit from 2016

The PoM units can be optional for CT2 and ST4 trainees in 2016, as these trainees will commence their mandatory units when they progress to their next training level i.e. ST3/5 in 2017.

Dear All,

The ops team will be notifying trainees of their August placements based on the information provided in the last couple of weeks by the TPDs. This will be in addition to the local mechanisms used by the Schools to notify their trainees. This is a mandatory process which HEE expects to ensure consistency across its processes. It also provides additional assurance to HEE that the Trainees are being notified 12 weeks prior to rotation date which is the code of practice followed by HEE.

We will be prefacing the rotation emails to trainees with a statement to the effect that this is in addition to comms they may have received already and may reflect only 6 month placement of their full year although they may have been placed for 12 months. This next 6 months will be reconfirmed again in the autumn. In addition, if they have received different comms/ information from TPDs/ School, they need to get in touch with us as changes may have been made recently that we are not aware of.  This will enable us to update Trusts and our records to reflect accurately the placements.

I appreciate that this is a new process for the Schools and trainees but it is not meant to replace the comms sent to them by the Schools.

I wanted to inform you ahead of us sending this information to reassure you that we are not placing trainees against TPD advice and any discrepancies may be due to recent changes that didn’t make its way to us or element of human error. But with 12 weeks to go we still have the opportunity to send relevant updates as necessary.

Professional Behaviour

Dear Trainees,

There have been some instances recently of trainees failing to attend non clinical aspects of their working week – notably departmental clinical governance meetings and departmental teaching at times they are supposed to be at work.

Clearly if these fall at times you are not supposed to be at work, attendance is voluntary and not expected. If these sessions fall during your normal working hours, you must attend unless you have booked leave or been in contact with your department. A failure to do so is a probity issue – essentially you are being paid to be at work but are absent. This is likely to be discussed at your ARCP.

This is always important but all the more so at this time when we need to support each other. I haven’t met any consultants who aren’t hugely supportive of your actions and understand the stress this places many of you under, professionally, personally and financially. If there are particular issues, please do talk to your College Tutor or Educational Supervisor, don’t just go AWOL.

Secondments

Dear all,

The GMC recently provided the College with feedback indicating that a significant number of Out of Programme Training (OOPT) applications were being withdrawn because the post applied for was in a location already approved for training. As a result we sought to clarify a list of approved locations in order to reduce the number of unnecessary applications and the associated workload. The GMC responded by producing a list of approved locations on their website which can be found at http://www.gmc-uk.org/education/28373.asp. The GMC have confirmed that this list will be updated fortnightly.

If a trainee wishes to go outside of the local training programme to an approved location then this would be considered a secondment rather than an OOPT. We have now created a Secondment Notification Form which can be found at http://www.rcoa.ac.uk/document-store/notification-of-secondment-form. I have also attached a copy of the form to this email. OOPT application forms will only be required when the post is in a location not listed on the GMC website. We understand that local processes may still require College approval for this secondment. As a result, approval has been delegated to the local College representative - the Regional Advisor. There is a space on the document for the Regional Advisor to sign to say that the post has been discussed with them and that they agree that the post forms part of a balanced training programme.

For all future secondments could you please send Secondment Notification Forms to alongside a copy of a job description so that there is a record of approval, as well as for our own data collection purposes.

Dr. Claire Sheppey

I was sadly informed today of the sudden and unexpected death of Dr. Claire Sheppey a Consultant Paediatric Anaesthetist and College Tutor at the Royal London Hospital.

Many Central School trainees have passed through the Royal London and worked with and been trained by Claire. Im certain she will be remembered fondly, not least for her often dark sense of humour.

PRISM QI Training resource

The NHS is no stranger to change, but most efforts at improvement have limited success. In order to improve the efficacy of our improvement projects, there is a growing understanding that clinicians should be leading change and to do this using quality improvement (QI) and implementation science methodologies. The Academy of Medical Royal Colleges is proposing QI forms part of a core curriculum of all doctors, and the RCoA curriculum annex G already contains QI knowledge and skills.

The perioperative improvement science and management (PRISM) website is a new online resource to help aid trainees, consultants and the wider perioperative team in learning and delivering QI projects. The website is designed around the needs of the perioperative team, and grounded in real life perioperative examples. The core of the website is a QI teaching module, completely based on and linked to the RCoA annex G QI curriculum, with a certificate of completion at the end. In addition, there are freely downloadable how-to-guides and templates that take you step by step through the process of QI, to help you with your QI projects.

Quality Improvement Survey

I am a fellow Anaesthetic Trainee currently undertaking an Education Fellowship at the Royal College of Anaesthetists with a specific remit to improve training and resources in Quality Improvement (QI) methodology for trainers and trainees in anaesthesia.

There is a current RCoA strategy to improve access to resources in QI training and aid delivery of QI training for trainers and trainees. To aid this we are aiming to collect information and feedback from trainees on Quality Improvement training in an attempt to recognise the efforts of trainees and improve learning resources available for them.

Joint Welfare Statement

Dear Colleagues,

It has been a difficult couple of weeks for doctors in training, in particular those working in the acute specialties.

The RCoA, FICM and the AAGBI agree it is crucial that trainees are, and feel, supported. Trainees are essential to the on-going safe delivery of anaesthesia and critical care; training and service are intrinsically linked.

We have been extremely concerned at the breakdown of negotiations between NHS Employers and the BMA, a decision to impose a contract and the looming threat of industrial action.

News yesterday of the Secretary of State’s intervention in the process is encouraging and we hope that this will help facilitate resolution of the dispute.

It is understandable that trainees may currently have higher degrees of stress, anxiety and uncertainty. Those studying for exams, preparing for job applications, interviews, presentations or just the daily challenge of making themselves better anaesthetists may really be feeling the pressure.

Consultants, SAS doctors and trainees alike must be particularly mindful of these pressures and the effects they can
have on individuals.

Don’t forget the support networks available if anyone feels overwhelmed:
1. Talk to somebody: fellow trainees, family and friends
2. Meet with your clinical or educational supervisor, College Tutor or mentor
3. Use one of the available support and welfare schemes:
a. AAGBI Support and Wellbeing
b. RCoA Career and Personal Difficulties
c. BMA Doctors’ Wellbeing

We are heartened by the support shown for trainees by consultants, SAS doctors and allied professionals.

We will continue to work with as many external bodies as possible to ensure that a safe and fair agreement is achieved in the interest of patients and for you the future of our specialty.

In the meantime, look after yourselves and each other.

OOP process with 6 month deadline

Dear Colleagues,

Health Education England (HEE) and English Deans (CoPMED) are keen to implement a consistent process across the LETBs with regards to the OOP submission process in order to allow sufficient notification to Post Graduate Deans as well as the programmes in order to manage gaps in training more effectively.  The requirement is now for submission of application at least 6 months in advance of OOP start date as recommended in the Gold Guide. What we propose to do in Anaesthetics is to require Trainees to submit an initial application form with information they have at hand to show intention of taking an OOP.  In recognition that full details and documentation regarding the OOP may not be available at this stage, we are expecting trainees to submit full supporting documentation 3 months prior to OOP start date- the same process we have in place at the moment.

Unfortunately, there is no choice in the matter but we hope that the expectation we have in place at the 6 month deadline will not disadvantage trainees in any way and still meet the requirements.  I have attached the guidance with updated deadlines for your information. The intended start date for the new process is Monday 5th of October 2015 for April 2016 OOP start dates. For the first month, i.e October, due to the short notice, we will accept these applications throughout October.

ARCP Requirements July 2015

Dear All,

This is just to remind people, as the ARCPS draw near, that evidence of having attended one of the simulation courses that NE/NC School offers each year will be expected. This has been a mandatory requirement for a year and without it, you are likely to get an outcome 5.

The Deanery has also imposed an additional mandatory requirement which you have probably been emailed about. You must present evidence of having obtained Child Safety Level 2 and Vulnerable Adult training. Mostly this will have been done as part of your Mandatory or Statutory Training at induction but you must bring evidence you have done it once in the past year. Again, you will be issued with an outcome 5 without it.

Most Trusts have these available as an e-module so there is still plenty of time to do it in your Trust.

Upper limit on dual training entry

Dear all,

Please be advised that potential Dual Anaesthesia and ICM trainees should be advised that from the August 2016 onwards, trainees will not be able to apply for Dual CCTs if they are beyond the end of ST5 in their initial specialty of appointment.

The August 2015 intake is therefore the last opportunity for trainees above ST5 to enter into the Dual programme (whether they be current anaesthesia only trainees intending to dual with ICM or ICM only trainees intending to dual with anaesthesia).

We would appreciate it if you could make any trainees in your region who may be affected aware of this change.

Mandatory Simulation Training

Dear All,

For the first time UCLP have funded a simulation course for every trainee per year of their training.
In order to allocate you to an appropriate course, we need to know which year of your training you are in as of 1st February 2015.

The proposed schedule is as follows, although we recognise that some of you have already attended some simulation courses. If this applies to you and you have already attended a course to which you are allocated, please do let us know.

CT1 - Novice and Critical Incident
CT2 - Obstetrics
ST3 - Paeds
ST4 - ACRM
ST5 - Airway
ST6/7 - Patient safety

Once you have replied with your year of training, you will be allocated a course which will most likely be at UCH, Whittington, Royal Free or Barnet. However, the course you are attending maybe at one of the Royal London sites.

We will try to give you lots of notice of the date of your course so that within reason you maybe able to swap if the date is inconvenient, however, if you are unable to swap it will be your responsibility to find another course which you maybe asked to pay for.

The school has decided to make attendance at your simulation day mandatory to pass your ARCP and evidence of your attendance will be sought.

New OOP Lead - Dr Robert Self

I have recently been appointed as the Out-Of-Programme (OOP) Lead for the School of Anaesthesia. This is a new position and will involve:

1) Discussion / advice available to trainees considering taking OOP posts.

2) Ensuring that relevant paperwork is completed for prospective approval.

3) Acting as a point of contact during the OOP time and involvement afterwards at ARCP.

I have been a Consultant Anaesthetist at the Royal Marsden Hospital since 2007 and before this was a trainee in the Central School of Anaesthesia. I have personal experience of the OOP process and speak on this topic at the RCOA ‘After the FRCA’ day.

The rules for OOP applications have recently been tightened by the GMC and they WILL NOT process apllications with less than the notice as specified below.

We have no control over this. Our new guidelines will match those of the other London schools:

6 months Intention to OOP discussed with TPD
5-4 months Completed paperwork, signed, approved and taken to school office by trainee
4-3 months School submits paperwork to Deanery/RCoA meeting deadline.

Study Leave Claims - Process has changed

School rules for ARCP

ARCPS are held in June/July and Jan/Feb each year. Most trainees need to be seen once a year but there are periods in your training when you might need to be seen six monthly. Ad hoc ARCPs are held throughout the year for trainees who cannot attend other dates.

Requirements for North Central School trainees are going to include an annual MSF from summer 2014. This is in line with most other Schools around the country.

Failure to have an MSF at the time of ARCP will mean an outcome 5 - incomplete paperwork - will be issued and a 2 week extension will be offered.

Out of Programme - Updated Process

We have updated the process to reflect that some documents such as job descriptions and College approval letters for time to count may come through after an OOP form is submitted.

The key parts of the new OOP process are:

• A fully completed and signed OOP form must be received by Operations Dept at least 3 months before start date
• OOPC can be submitted with shorter notice for unplanned and urgent reasons
• Outstanding documents will be requested by the Operations Dept with a deadline date – if information is not provided, the OOP application will not be approved
• The process will start from 7 April 2014 deadline for applications for a July OOP start date.

Local Education Training Boards

Dear All,

As many of you will have heard, all postgraduate training in anaesthesia since April has been managed by Local Education Training Boards or LETBS. Almost as soon as they came into being, their name changed to Health Education England, in our case North East North Central (HEE NE/NC). They commission training in anaesthesia in the North Central and North East areas of London, essentially in the areas and hospitals which are part of the North Central School (the ‘Central’ School’) and the North East School of Anaesthesia (’Barts and the London’ School) .

The delivery of training is still done at each hospital or Local Education Provider but a middle man called, in our case, UCLP or UCL Partners, now manages the interaction between the commissioning and the local provision. Recruitment, the ARCP process, OOPE/T applications and much of the admin associated with training still sits with the Deanery which is now called the London Shared Services. That means you will receive communications from UCLP and from Shared Services and from the Schools and there are a myriad of courses and training opportunities available so this should be good for your training overall.

Over the next year, we will work to share best practice across both Schools and to look at how to share training opportunities and educational resources of all sorts. We will be meeting regularly with your trainee reps and again, we hope this will be good for trainees and trainers alike.

Day to day, nothing will change for any of you, except possibly get better!

Dr Khalid Rauf RIP

Dr Khalid Rauf, consultant anaesthetist, died suddenly on the 11th of August. He was 52.

Khalid joined The Whittington in 2006, and whilst you may not recognise his name, many of you will recognise his face, as he saw patients in many areas of the hospital.

Khalid was a quiet man, who was known for his calm, gentle and considerate manner and his ability to allay the fears of the most nervous patient.

Outside of work, Khalid loved his family and cricket. He was married with three sons, the oldest of whom will start at St Georges Medical School in the autumn. His second son recently completed work experience here at The Whittington, with a view to also becoming a doctor. His youngest son is eight years old.

Khalid was buried on the 13th of August, in accordance with the Muslim faith. There will be Remembrance service on Thursday 12th of August at 2 p.m. in the Whittington Education Centre, to which all are invited to come and pay their last respects. A book of remembrance has been opened, and can be found at the reception desk in Main Theatres.

Guide for Novices starting in Anaesthesia

The Royal College of Anaesthetists has produced an interactive Guide for Novice Trainees specifically designed to support the first three to six months on the training programme.

The Guide, available in the form of a 4GB USB memory stick, has been sent out to all CT1 and ACCS Anaesthesia trainees starting in August 2013.

It contains key documents and a step-by-step guide to help trainees get started on the training programme. It also contains all 82 e-learning sessions from Module 1 of e-Learning Anaesthesia, specifically written for Novice Trainees and additional learning resources/guidelines from the AAGBI, Resuscitation Council and Difficulty Airway Society.

An on-line version of the Guide for Novice Trainees is available via the link below as well as instructions on how to install and start using the Guide from the USB stick.

The on-line version is compatible with most tablet computers and smart phones. The introductory video and videos/animations contained in some of the e-Learning sessions require Adobe Flash and therefore can only be viewed if using a Flash compatible browser on these devices.

KSS Deanery Bursaries

KSS Deanery Bursaries for PG Cert, PG Dip Anaesthesia and Perioperative Medicine, opportunity for London trainees.

20 bursaries towards the cost of course fees for either the Postgraduate Certificate or the Postgraduate Diploma in Anaesthesia and Perioperative Medicine, run by the Brighton and Sussex Medical School and validated by the University of Brighton.  The bursary will provide £1,500 towards the overall cost of the course.  The fees for UK / EU students are as follows:

PG Cert (3 modules over 1 year): £2,130
PG Dip (6 modules over 2 years): £4,260 (subject to a small increase in 2014/15 in line with inflation)

The first 20 successful applications, to be received by 31st July, will be offered a bursary. (Applications for the course may be made after the 31st July but will not be considered for a bursary).

Extension Training/Grace Periods

The GMC recently issued clarification over their interpretation of extension periods (sometimes known as grace periods).

They can be offered at end of CT2 (or CTb for ACCS) for lack of primary (or lack of other core competences).

They can also be offered at ST4 for lack of final or lack of (non-exempted core competences).

However, because it is recognised that trainees may gain competences at
different rates for a number of reasons, trainees will be able to have
additional aggregated training time of normally of up to one year within the
total duration of the training programme in the hospital specialties and
normally up to six months in general practice because of the short duration
of the training programme, unless exceptionally, this is extended at the
discretion of the Postgraduate Dean, but with an absolute maximum of two
year additional training during the total duration of the training programme.
This does not include additional time which might be required because of
statutory leave such as ill health or maternity leave. Assuming that the
trainee complies with the additional programme that has been planned, this
enables reasonable time for the trainee, but does not unduly disadvantage
other trainees who may be attempting to gain admission into run-through
training in the specialty. If the trainee fails to comply with the planned
additional training, he/she may be asked to leave it and the training
programme before the additional training has been completed.

This is taken to mean that you may have a year as a CT2 and/or a year at ST4.
(But you cannot have over a year at either on its own - eg you can’t have 18 months extension in ST4).

There have been previous arrangements that pre-dated this that will be honoured.

GMC publish review of Medical Training in London

Medical training in London is good, but the quality of supervision and clinical placements varies
Doctors in training and medical students in London are being well educated and trained, according to our new report.

But Regional review of medical education and training in London: 201213 shows some areas of concern, particularly around the quality of supervision and clinical placements, need to be addressed.

The findings come from visits to the local education providers, medical schools and the deanery* in London over three months in 201213. The report gives a valuable insight into the experiences of almost 12,000 students and doctors in training 20% of the total in the UK.

Dr John Ruston RIP

We deeply regret to announce that Dr John Ruston died unexpectedly in his sleep on the 21st of January. He was 57 years old.

John had been a Senior Registrar at the Royal Free and was appointed as Consultant in 1993. He was a versatile and knowledgeable anaesthetist, able to handle a wide range of clinical cases with great skill and confidence. He was calm, methodical and completely reliable.

He was much-loved by all who knew him, popular with staff and with patients, and was always a source of common-sense advice and a good friend.

He is survived by his wife Mandy and his two sons. We are awaiting details of the funeral arrangements.

Time limiting of CCT applications

Please see the information below from the GMC.

As you may be aware the Postgraduate Board has approved the introduction of a limit on timeframes within which a doctor is able to apply for a CCT / CP. The limit will be 12 months from the doctors expected end of training date, and will be effective from 31 March 2013.

We are now in a position to communicate this key change to current and future trainees in order for it to be implemented successfully. The GMC guidance pages on the internet have been updated fully and I have enclosed a link to these pages for your convenience.

E-Portfolio

The London Academy of Anaesthesia Board has agreed that it is mandatory that you use the e-Portfolio for training placements from 1 August 2012, if you are entering the following years of training:

CT2
ST3
ST4
ST5

During your ARCP at the end of your training year you will be expected to present all evidence via the e-Portfolio. Paper based workplace based assessment will not be accepted (with exception of ICM paperwork which currently remains paper based).

If you are starting your ST6 or ST7 year in August 2012, you may use the e-Portfolio but it is not currently mandatory.

BLT and ILT certificates and ILPR

At the ARCPs I rejected a lot of certificates which were presented for my signature because they were incomplete.
The whole certificate MUST be completed by either a College Tutor or ES, not just the front page. All sections must be countersigned by one of these trainers to confirm that, for each of the modules, the evidence has been seen to satisfy completion of the module.
If an ST4 trainee is on the 2010 Curriculum and has either Cardiac or Neuro to carry over into ST5 they need an Intermediate Level Progress Report.
So apologies for being pedantic, but I will not sign certificates if they are not fully completed.
College Tutor at the trainees’ current placement can do this, even if some modules were done elsewhere.
It is a bit of work checking that everything has been completed, but if other ESs have done their bit by accurately filling out the ES report required for the ARCPs, then it should not be too much trouble to get the details about when modules were completed.

Dear Trainee

At the recent ARCPs we tried to run a separate panel for e-portfolio candidates.

It was not overly successful.

Discussing the issues with the trainees I have found

· Little buy in from non Core Trainees

· Inconsistency throughout trusts regarding the amount of consultants who have registered

· A reliance on paper portfolios

From August 2012 we expect all trainees to use e-portfolio and future ARCP events will be conducted with this.  Hence, we would like all WPBAs, etc to be performed via the e-portfolio system from now on.

For those not registered this must be rectified as soon as possible.  Contact Georgina, the School Administrator () with your email, College number and GMC reference number.

College Tutors have been alerted to this and instructed to ensure all Consultants are registered with the e-portfolio system.

At the last ARCP there were >75% outcome 1s.  In the future we will know who these are and may either not call them to be seen or spend the time planning rotations or offering advice.

Messages do not appear to have been transmitted, so in order to avoid any further misunderstandings I would like to clarify what we expect as a School.

There are 2 things running in parallel when considering the number of WBA required.

1) The minimum WBA required to demonstrate that you have satisfied the minimum learning outcomes for a module.

This is where it is necessary to provide a minimum of 1 of each type of WBA per module. Ideally you would have more, as you would like to demonstrate that you are developing within that module, not just achieving a minimum standard.

2) In addition you should be doing WBAs to demonstrate the achievement of all sorts of other things in the curriculum including Professionalism, for example.

So you should not just do the bare minimum, but demonstrate an engagement with the training process and do:

12 DOPs, 8 A-Cex and 4 CBDs for a year. If you are only in a 3 month placement, you should come away with 3 DOPS 2 A-Cex and 1 CBD. This is just a little bit more than the minimum requirement for a module.

If you are somewhere for a year keep the latter in your mind as a target for each 3 months.

In addition an MSF is now required every year.
Your ARCP is your documentation for your revalidation. So it is an important process.

Ideally this is all done on the e-portfolio, and we expect all trainees in years 1-5 to only use e-portfolio next year. If using paper all modules must be signed off on the completion of Unit of Training form (Available either on School website under Forms and Downloads or on College website) which should also state the evidence used in forming the decision that you have achieved the minimum standards.

These guidelines will be being reviewed periodically according to College and/or Deanery requirements. I will ensure you get updates via this website.

This is an excellent way of utilising one of their funded 10 KIT days (Keep In Touch days) to refresh core skills before they start back at work, as recommended in the May 2012 AoMRC guidance on “Returning to Practice”.

Changes to Higher Training and CCT

The GMC has decreed that if you accredit after August 15th 2012, you must have higher competencies in obs, cardiac, paeds and neuro w 12 months general duties (often time can be double counted depending on where you acquire these competencies) and 3/12 higher ITU signed off. If you do not, you may find your CCT extended.

If you cannot identify how you can get this, you need to see or speak to Dr Lila Dinner or Dr Alan McGlennan soon. Do not panic - there are ways to sort this out and many of you, on examining your rotations, will be fine with some minor form filling to do. However, a number of you will need to adjust your rotations. There is no way around this. We have not been given very much notice of this deadline. Forms for higher competencies are available from the school or RCOA websites - completion of unit of training (2010 curriculum). This applies whether you are on 2007 or 2010 curriculum.

In light of this, I have started to make some changes so please do check your rotation first.

Completion of Unit of Training Certificate - A College Clarification

The RCoA have recently attempted to clarify the CUTC.  Here is a statement that has gone to all Schools.

The purpose of the Completion of Unit of Training Certificate [CUTC] is to confirm that all the requirements for each unit of training attempted have been met, ie, case mix, assessments and most importantly, that the trainee has achieved the core clinical learning outcomes. In other words, the trainee has met the minimum standard. This means that a separate CUTC must be completed for each unit in the Basis of Anaesthetic Practice [BAP] and for each of the units in general duties. The IAC only meets the assessment requirement of the BAP. This applies equally to ACCS trainees.

The CUTC plays an important role in the next phase of the e-Portfolio. It is the generation of this certificate which is used by the system to show that a unit of training has been satisfactorily completed, ie turning the UoT green and then feeds into the Educational Supervisors Structured Report for ARCP.

Assessments and Competencies - which do you need?

There has been a bit of confusion about this, particularly when it comes to RITA’s. The bottom line is that you probably need both, particulary if you will still be in year 1 or 2 in February 2005.

Assessments are carried out on a per-hospital basis, and are normally carried out when your rotation at that hospital ends. Your assessments will be inspected at a RITA. Normally, you will not have been assessed at the hospital where you are presently working, unless your departure is imminent.

SpR 1/2 Competencies are subspecialty-based (eg ENT, Paediatrics, Pain). You can get them signed in any order, and at any hospital. In order to enter year 3 of training you will need to have obtained signatures in the 7 “key” modules (although there is a 6 month period of grace under certain circumstances).

Logbook for trainees in Pain Medicine

A new Pain Medicine logbook has been kindly and diligently put together by Roger Laishley and Barry Miller. 
The logbook has been developed as a standalone record using a spreadsheet model and will work with Microsoft Excel, OpenOffice or iWork. There is an FAQ included for further information.

I would like to draw your attention to the SOCIETY OF BARIATRIC ANESTHETISTS (SOBA) UK. http://www.sobauk.com
As I am sure you are all increasingly aware, we have to manage obese patients on a daily basis in all sub-specialties. This can sometimes be a daunting prospect. SOBA run a one-day meeting three times a year entitled How to Anaesthetise the Bariatric Patient, although mainly attended by consultants in the field, this is an excellent way of obtaining the tips to manage even the bariatric patient for non-bariatric surgery.
The RCOA is currently planning new college training competencies in bariatric and obesity anaesthesia. SOBA is involved in this process, so if you want to be ahead of the game, come visit our website.
Bariatric Surgery is a rapidly growing field and many hospitals are considering establishing or increasing a bariatric service. In terms of future career planning, this field is one you should really be aware of. Whilst many trainees have little exposure to bariatric anaesthesia training, SOBA members are able to access a directory of day-visit centers, happy to provide a taster of the specialty.
There are training regions that have Bariatric Anaesthesia Fellowships. I myself am undertaking one currently and it is an excellent opportunity to gain transferable skills. If you are interested in this, you should contact our Fellows Representative at .

Perioperative Medicine Academic Training Forum

We have established a trainee-led perioperative research training group. We are organising a pan-London conference for all those involved in the perioperative medicine research translational pathway. This conference is aimed at scientists to clinicians, basic researchers to clinical researchers, medical students to consultants, and BSc students to professors.
The aim of our group and this conference is to improve and promote research training, networking and collaboration within our specialties, and ultimately the beneficial translation of this research to our patients.

2010 Curriculum Implementation

The GMC have approved the Colleges implementation plan to transfer trainees currently on the existing 2007 GMC approved CCT in Anaesthetics curriculum to the 2010 CCT in Anaesthetics curriculum. In approving the implementation plan, the GMC has recognised that the training at the basic and intermediate levels in the two curricula [2007 and 2010] are equivalent and therefore trainees who transfer will be eligible for a CCT on successful completion of the UK approved programme. In addition, the GMC has agreed transitional arrangements for the recognition of examination credits accrued prior to transfer to the new curriculum.

Implementation

The implementation plan for the 2010 curriculum is as follows:
All CT1 trainees starting from August 2010 will be appointed to a programme using the 2010 curriculum.
All CT2 to ST5 trainees will be transferred to the 2010 curriculum in August 2010 or all CT2 to ST5 in August 2011.
The decision of when to transfer to the 2010 curriculum [August 2010 or August 2011] lies with the School of Anaesthesia and not with an individual trainee.
All trainees starting ST6 and 7 in the year the School transfers CT2-ST5 trainees to the 2010 curriculum will remain on the 2007 curriculum.
The 2007 curriculum will continue as an approved curriculum until midnight, 31 December 2015 to allow those less than fulltime ST 6 and 7 trainees to complete their CCT.
Some limited flexibility in deferring ST3/4 units of training to ST5 will be available for ST4 trainees for the year of transfer.

Transitional arrangements of examination credits for implementation of 2010 curriculum

Transitional arrangements have been agreed with the GMC for those trainees who were appointed to a CCT programme in Anaesthetics prior to 1 August 2010 and who are transferred to the approved
2010 curriculum. For trainees who have completed the respective Irish examination in its entirety [i.e. the Primary and/or Final] by the date of transfer to the 2010 curriculum, the examination will be
recognised for the award of a CCT.
For those trainees who have partly completed the Irish examination, such as the MCQ component only, on transfer to the 2010 curriculum, there are two options:
Complete the Irish examination and proceed with the training programme for the award of a CESR[CP]; or
Complete the RCoA examination [within the rules for number of permitted attempts and exam validity]

Please note, the Irish examinations are currently not approved tests of knowledge for the award of a CCT for the 2010 CCT in Anaesthetics curriculum. If a trainee eligible for a CCT after transferring from
the 2007 curriculum completes an Irish examination after 1 August 2010 or 1 August 2011 (depending which year the School transfers to the new curriculum] in lieu of the respective UK examination, the trainee will lose their eligibility for a CCT but will be eligible for the CESR[CP] instead.

Calling all anaesthetists. ‘Music from the genome’ events.

Some of you may be already aware of the ‘Music from the Genome’ research pr=ject. It is being directed by Andrew Morley, a consultant anaesthetist at =uy’s and St Thomas’ Hospitals, and is funded by the Wellcome Trust. It com=ines a study of the genetic determinants of musical ability (comparing the=DNA of 250 choral singers with that of 250 non-musicians) with the creatio= of a new 40-part unaccompanied choral work for the New London Chamber Cho=r (NLCC), entitled “Allele”. Many of the “non-musician” participants in th= genetic study were recruited at anaesthetic events in London and the Sout=.

“Allele” is being written by Michael Zev Gordon (winner 2008 British Compos=r of the Year choral section) and Ruth Padel (well-known poet and Charles =arwin’s great great grand-daughter). Michael has used the results of each =ndividual singer’s genetic analysis to create their part. The NLCC singers=will effectively be singing their own genes. 

There are a few project related events between now and summer and the first=is at the Southbank Centre on Wednesday March 3rd. This is part of their s=ries of talks on Music and Writing. It will feature a short introduction f=om Andrew about the science behind the piece and then a conversation betwe=n Andrew, Michael and Ruth, chaired by the writer and academic, Eva Hoffma=. Michael has written about of the piece already and there will be a f=w excerpts for discussion, recently recorded by the choir. The whole thing=will last about 90 minutes and the link is below.

The piece itself will be premiered at Diamond Light, the UK’s National Sync=rotron Facility in Didcot, on the evening of Fri 9th July. This will be as=part of an NLCC concert to be held in the atrium of the Synchrotron buildi=g. The New London Chamber Choir are top notch, but pretty uncompromising c=ampions of contemporary music. Traditionalists, however, will be happy to =earn that the programme includes Spem in Allium (Tallis), together with Ma=er Christi (Tavener), Sanctum est Verum Lumen (Gabriel Jackson), Libera No=. 1 & 2 (Sheppard) and Lux Aeterna (Ligeti). The concert will be preceded =y a bookable tour of the facility itself. It’s pretty awe-inspiring - a so=t of mini-CERN, visited for inspiration by the producers of one of the mor= recent Bond films and the latest Dan Brown. There will be a Q&A afterward= with Andrew, Michael and James Weeks, the choir’s musical director.

On Saturday 10th July, the NLCC will be doing a similar concert, featuring =llele, at Cheltenham Music Festival in Cheltenham College Chapel, with a s=ort pre-performance introduction from Andrew about the science. Tickets on=sale via the website from early April. The final, London performance will =e at the end of a Music from the Genome evening at the Royal Society of Me=icine on 13th July under their public engagement banner. Marcus du Sautoy,=Oxford Professor of the Public Understanding of Science will open the proc=edings with a lecture on “How science feeds the arts” or similar. Thereaft=r some short talks from Michael, Ruth and Andrew - the choir will do the T=llis and finally, Allele. Entry free.

New Study leave form for ST3+ Trainees

We have been sent a new Study Leave claim form to be used by ST3+ Trainees.
It can be downloaded from http://www.schoolofanaesthesia.co.uk/downloads/ST3+%20Study%20Leave%20Claim%20Form%20revised%20December%202009.zip.

Your study leave budget is Currently 750 per year from August - July. To make a claim you will need to send the fully completed form, together with your College TUtor signature, original receipts and your bank details to Anne Ramlal at St Thomas Hospital.

Intermediate Level Training Certificate - ST4/5 should read

It has come to my attention that the Intermediate Level Training Certificate has not been claimed by many trainees.

This is the piece of paper which certifies that you have satisfactorily completed the Key Units of Training and passed the Final FRCA. It can be filled in by your College Tutor. The College requires this to calculate your final CCT date.  Please note that until then your date is only provisional.

Safeguarding Children - message from the President of the RCoA

September 2009

Dear Regional Adviser and College Tutor

In July 2009 the Care Quality Commissions (CQC) review of arrangements within NHS Trusts for safeguarding children and young people revealed significant gaps in the uptake and monitoring of training. Their current recommendation is that all anaesthetists should complete Level 1 and 2 training in Child Protection.

Safeguarding Children and Young People is the new e-learning program developed by the Royal College of Paediatrics and Child Health (RCPCH). The programme is designed to help health professionals meet the statutory training requirements of Section 11 of the Children Act and ensuring people are up to date with the issues surrounding Child Protection. The programme is closely aligned to current best practice as set out in the Intercollegiate Document 2006.

Level 1 - Introduction to Safeguarding Children and Young People:

A single session that covers the knowledge and competences required for Level 1 Safeguarding

Level 2 Recognition, Response and Record:

Three sessions that cover the knowledge and competences required for Level 2 Safeguarding

These sessions are now available on e-Learning Anaesthesia (e-LA) to ALL anaesthetists.

If you are already using e-LA you can log on with your existing username and password

If you havent yet registered for access to e-LA please visit http://www.e-LA.org.uk and follow the link to the registration page. You will be sent your username and password within few days

Level 1 and 2 training can be found on the Learning Management System by clicking the menu item Mandatory on the left of the screen as shown in the image overleaf. A short PowerPoint presentation with details of how to access this is also available on the College website.

The College recommends that all anaesthetists complete the National Level 2 e-Learning package to support local implementation of the current guidance from the CQC.
